The Avalon's goal and services are based on two fundamental philosophical principles. The belief in the innate worth of the aged and disabled individual and the belief that each individual, regardless of age, sex, race, religion or handicap, is entitled to maximize his/her potential as a human being and be an active member of society.
It is the Avalon managements belief that the aging process is a normal step in the development of any individual and that chronic disease and disability are, to some degree, a part of the process. The Avalon is dedicated to assisting residents to maximize their contributions to society, fulfill their goals as family members while living comfortably in a congregate setting and receiving personal care assistance as necessary, thereby promoting individual respect.
DESCRIPTION OF FACILITY
* The 2-story facility can accommodate a maximum of 76 residents.
* All rooms are spacious and large enough to accommodate two residents.
* Private room arrangements are available.
* Each floor has its own dining room, and all meals are served in a congregate setting. In addition, each floor has a lounge area, public restrooms, recreation area and a spa room.
* A beauty salon/barber shop is located on the ground floor.
* A fully authenticated elevator as well as two staircases connect the ground floor to the second floor.
* Outdoor areas are beautifully landscaped and include a gazebo area as well as a sitting area.
* The facility is located on a main thoroughfare (Rte. 376) near a small airport (Dutchess County Airport) within minutes of two major area hospitals, as well as the Poughkeepsie Galleria Mall, the Cross Court Tennis Club and the Red Oaks Mill area.
SERVICES AVAILABLE & THE AVALON
All services at The Avalon are licensed by the New York State Dept. of Health. In addition to the basic license, we are also licensed to provide Assisted Living Program care (ALP). This additional license enables residents of the facility to age in place, and to receive additional hands on care as they become more frail and in need of assistance with daily living. In accordance with the two licenses, the facility is able to offer the following services:
* Full room and board
* 24 hour supervision - Intercom and call bells are connected to a central station.
* Meals - are nutritionally planned by registered dieticians. Special diets are served.
* Life enrichment and diversified activity - plans include trips to local attractions.
* Personal Care Services: Highly trained and certified personal care aides provide 24-hour assistance with all aspects of personal care such as bathing, dressing, eating, grooming and ambulation.
* Housekeeping Services: Daily room cleaning, including making beds and laundry
* Medication Management: Highly qualified and trained personnel assist in the administration of the medication. All meds are centrally stored and administered in accordance with the physicians’ orders.
* Case Management: All aspects of care and services are supervised and coordinated by a Registered Nurse.
* Accessibility to physicians and other professional providers: Space and accommodations are made to professional providers who visit the facility. Such professionals include primary physicians, podiatrists, physical therapists, occupational therapists, speech therapists, audiologists, etc.
* Rates: Fees for our services vary, based upon individual needs. Our administrative staff will be happy to discuss services, fees and the availability of potential funding sources. Medicaid funds are available for residents who participate in the Assisted Living Program (ALP).
