Adult Home/Assisted Living Program

Mount Alverno Center is a New York State approved Adult Home with an Assisted Living Program serving 85 residents. Our goal is for each resident to maintain a maximum level of independence. Individualized care plans are designed to help residents reach their utmost daily potential. Services are based on a team approach and are provided by highly competent professionals dedicated to the comfort and safety of our residents.

Set in park-like, beautiful, country surroundings, Mount Alverno Center offers a host of professional services in a choice of private, semi-private rooms or one-bedroom suites. Mount Alverno Center provides a gentle helping hand with daily activities, medication monitoring and nutrition.

Affordability a low monthly fee (no entrance fee is required) provides you with a private or semi-private room and personal care services. Additionally, there are a limited number of one-bedroom suites available. When you compare the monthly fee with the costs of maintaining a personal residence and obtaining the same quality services and care provided at Mount Alverno Center, the fee is very affordable.Application ProcedureTo apply for residency at Mount Alverno Center please complete the Application for Admission Form and return it to the Administrator. LEVEL OF CARE CRITERIA

Adult Home

Level I

* Independent in ADL’s
* Self directing
* Completely self medicating
* Continent

Adult Home

Level II

* Requires minimal assistance with one or two ADL’ s
* Meal setup (opening containers, cutting meat, etc)
* Bathing setup; stand by assistance
* Continent or self manages incontinence
* More difficult parts of dressing ( preparation of clothing, buttons, footwear, etc)
* Blood sugar monitoring, Pacemaker checks
* Staff Assistance with oral medications, eye drops or nebulizer treatments
* Verbal cueing to change clothes, bathe, etc
* Occasional direction/redirection

Assisted Living Program

* Requires assistance with two or more ADL’s, meal setup and monitoring for completion
* Staff showering/bathing
* Help with toileting, transfers
* Frequent verbal cueing
* Frequent direction/redirection
* Staff assistance with all medications, blood sugar/pacemaker check
