The Oaks at Fort Hudson
The Oaks, a senior retirement community, offers 62 spacious apartments, beautifully appointed common areas, expansive grounds, and a friendly atmosphere. Situated on the Fort Hudson campus, additional benefits include multiple health-related services in one convenient location and peace of mind. It's the "comfort of senior living".

Sandy Hill Pavilion - Specialized Dementia Care Unit
At Fort Hudson, we understand that people living with dementia or Alzheimer's disease have unique needs. Our state-of-the-art Dementia Care Unit combines specially trained staff and specialized programming to help maximize the functional, social and cognitive abilities of our residents.
Our secure, home-like environment is complete with such amenities as wandering paths, "way finding windows," therapeutic gardens, sensory boards and continuous corridors to enhance our residents' comfort and facilitate their movement throughout the facility.
Our goal at all times is to provide meaningful activity and compassion, while ensuring residents the highest degree of dignity and independence.

Welcome to Fort Hudson – the leading senior health provider throughout the greater tri-county area. For 40 years, Fort Hudson has recognized that individuals seek choice when selecting healthcare options. For that reason, Fort Hudson offers comprehensive adult services to seniors of our community.
Fort Hudson Day Services goal is to assist in maximizing independence and quality of life by offering a day center option for those residing in the community. We embrace the philosophy to change how you live, not where you live. Programs available through Fort Hudson Day Services include:

Fort Hudson Day Services offers Monday – Friday coverage
8:00 am to 4:00 PM. Our programs recognize that individual and caregiver routines vary. We offer early and late pickup, as well as full and part time options to accommodate you.
Our staff, comprised of registered nurses, licensed practical nurses, certified nurse aides, personal care aides, and home health aides are proud members of the Fort Hudson team. Professional and paraprofessional support is also available offering therapies, case management, and nutritional counseling to maximize one’s health and wellness.
We understand the importance of our participants and caregivers. Our programs encourage a stimulating, and social environment for the participant, while allowing a caregiver to pursue work, school, vacation, or errands with peace of mind. Decorated in a comfortable Adirondack lodge theme, it is an inviting place to laugh, relax, and socialize with friends.
SOCIAL PROGRAM
Our Social Program is a perfect option for those seeking a supportive day center, and do not require ‘hands-on’ clinical care. Services include:
* Meals – breakfast and lunch

* Medication Reminders
* Personal Care Cueing
* Safety Monitoring
* Peer Companionship

MEDICAL PROGRAM
Individuals with short-term or chronic health conditions will benefit from Fort Hudson’s long- standing adult day healthcare program. Services include those listed above, in addition to the following:

* Oxygen Therapy

Sometimes it’s a challenge to know how to determine what program would best serve an individual. Fort Hudson is proud of our Single Point of Entry approach to all programs on our campus. Our screening process provides one-to-one attention, working with individuals, to identify the best program fit for each person’s needs.
Fort Hudson Nursing Center
For over thirty-eight years, the Fort Hudson NursingCenter has served as the cornerstone of our campus. Our facility, offering 24-hour care, ensures that individual's medical, physical, cognitive and psychosocial needs are met with compassion and the highest clinical standards. Residents and families of the nursing center are made to feel a part of the Fort Hudson community, and are encouraged to participate in the daily life of our residence.
